This is a tool to facilitate communication between the patient, the physician and the pharmacist. A set of three copies (6 pages) should be printed on both sides to keep a proper format.
The COPD Action Plan co-developed by the CTS (COPD Dissemination and Implementation Committee), Family Physicians Airways Group and Living Well with COPD.
To date, the use of the COPD Action Plan has only been evaluated and validated in a randomized controlled trial with the support of a respiratory educator. Ideally, it is a clinical tool for use between a patient and a health care professional who has adequate experience and time to properly educate the patient on how best to use the Action Plan. The health care professional should also be available as a resource in the future to collaborate with the patient in the implementation of this self-management tool. The health care professional could be a physician (family physician or specialist) or ideally, where available, a certified respiratory educator (CRE).
Input continues to be gathered from family physicians, respiratory educators and pharmacists. As such, the action plan is still deemed to be in its evaluation stages.
