This is a tool to facilitate communication between the patient, the physician and the pharmacist.  A set of three copies (6 pages) should be printed on both sides to keep a proper format.

The COPD Action Plan co-developed by the CTS (COPD Dissemination and Implementation Committee), Family Physicians Airways Group and Living Well with COPD.

This action plan was developed as a pilot project to be tested within respirology clinics and pulmonary rehabilitation programs with a certified respiratory educator present to review the plan with the patient.

Input continues to be gathered from family physicians, respiratory educators and pharmacists. As such, the action plan is still deemed to be in its evaluation stages and should only be administered by a certified respiratory educator.
